What is an Addendum?

Overview

An Addendum is additional documentation attached after a clinical note has been signed. It preserves the original signed note while allowing necessary clarifications.

When is it used?

Use an addendum when:

  • You need to clarify information after signing
  • New relevant information becomes available
  • Clinic policy requires corrections via addendum instead of editing signed content

How to add an addendum

Procedure

Before you start

  • The original note is signed
  • You have permission to create addendums
  • You know what clarification to document

Steps

  1. Open the signed clinical note.
  2. Select Add Addendum.
  3. Enter the addendum text.
  4. Save and sign the addendum if required by your workflow.
