What is an Addendum?

An Addendum is additional documentation attached after a clinical note has been signed. It preserves the original signed note while allowing necessary clarifications.
When is it used?
Use an addendum when:
- You need to clarify information after signing
- New relevant information becomes available
- Clinic policy requires corrections via addendum instead of editing signed content
How to add an addendum
ProcedureBefore you start
- The original note is signed
- You have permission to create addendums
- You know what clarification to document
Steps
- Open the signed clinical note.
- Select Add Addendum.
- Enter the addendum text.
- Save and sign the addendum if required by your workflow.
